The following documents must be presented by the Funeral Home to the Consulate of Costa Rica for authentication:

• Original Death Certificate
• Burial Removal Transit Permit
• Letter of No Contagious Disease from Health Department
• Embalmer’s Affidavit
• Embalmer’s Report
• Copy of deceased’s passport
